Vitamin B12 Deficiency: When the Problem Isn’t Getting Enough—It’s Absorbing It

Vitamin B12 is often associated with energy, but its role in the body reaches much further. It is essential for healthy red blood cell formation, DNA synthesis and normal nervous-system function—and because the body can store a substantial amount of B12, a deficiency may develop quietly for a long time before symptoms become obvious.
For some people, the issue is simply not getting enough B12 in the diet. For others, the problem is more complicated: they may be eating plenty of B12-rich foods but not absorbing the vitamin efficiently. Understanding that distinction can be an important part of figuring out why fatigue, neurological symptoms or other unexplained changes are occurring.
Why is vitamin B12 so important?
Vitamin B12 is a water-soluble vitamin that works as a cofactor in several important metabolic processes. It is involved in DNA synthesis, red blood cell production and the development and maintenance of the nervous system. B12 also participates in the metabolism of homocysteine and in pathways involved in the production of methionine and other cellular compounds. That means a deficiency can affect more than energy levels. People with B12 deficiency may experience fatigue or weakness, but they can also develop numbness or tingling, problems with balance, memory or concentration, anemia, or other neurological changes.
One important point is that neurological effects can occur even without the classic anemia associated with B12 deficiency. That is one reason it is worth looking beyond a single symptom—or a single laboratory number—when B12 deficiency is suspected.
How does the body absorb vitamin B12?
This is where B12 gets surprisingly complicated. B12 found naturally in foods is attached to protein. During digestion, stomach acid and digestive enzymes help release the vitamin from that protein. B12 then binds to intrinsic factor, a protein produced by cells in the stomach. The B12–intrinsic factor complex travels to the small intestine, where B12 can be absorbed. This multi-step process explains why someone can consume an adequate amount of B12 and still become deficient.
Problems with stomach acid, intrinsic factor, the stomach lining or the small intestine can interfere with absorption. Pernicious anemia, for example, is an autoimmune condition in which the body cannot produce enough intrinsic factor. Celiac disease, Crohn’s disease, certain gastrointestinal surgeries and other digestive conditions can also increase the risk of deficiency. This is also one reason B12 deserves a closer look in people who have ongoing digestive problems.
Who is most likely to become B12 deficient?
Diet is certainly one factor. Vitamin B12 occurs naturally in animal foods such as meat, fish, poultry, eggs and dairy products. People who eat little or no animal food need to obtain B12 through fortified foods or supplementation. Age can matter, too. As we get older, changes in stomach acid and the digestive system can make it harder to absorb B12 from food. The NIH specifically recommends that adults over 50 obtain most of their B12 from fortified foods and/or supplements because the body may not absorb food-bound B12 as efficiently.
Certain medications are another piece of the puzzle. Long-term use of acid-suppressing medications, including proton pump inhibitors and H2 blockers, can interfere with the release of B12 from food. Metformin can also reduce B12 absorption and lower blood levels. And then there are people with conditions that affect the stomach or small intestine, those who have undergone gastrointestinal surgery, and people with pernicious anemia or other causes of malabsorption. Sometimes there isn’t one single reason. Several relatively small factors can add up.
What does B12 deficiency feel like?
B12 deficiency can develop gradually, and early symptoms can be easy to dismiss. Fatigue and weakness are common possibilities. Some people experience difficulty concentrating or changes in memory. Others notice numbness or tingling in their hands or feet. Anemia can cause weakness, shortness of breath or palpitations. More significant deficiency can cause neurological problems, and those symptoms deserve attention rather than being automatically attributed to aging or stress. The NIH notes that B12 deficiency can damage the nervous system even when anemia is not present. Of course, these symptoms have many possible causes. Fatigue alone does not mean someone is B12 deficient. That is where appropriate testing becomes important.
Is a normal B12 test enough to rule out deficiency?
Not necessarily. A serum B12 test is commonly used to assess B12 status, but interpretation can become more complicated when the result falls into a borderline range or symptoms and laboratory findings don’t seem to match.
Methylmalonic acid (MMA) is a particularly useful additional marker because it rises when B12-dependent metabolism is impaired. Homocysteine can also rise with B12 deficiency, although it is less specific because other factors—including folate status and kidney function—can affect it.
Current NICE guidance also emphasizes that B12 testing should be interpreted in context and recommends obtaining diagnostic blood samples before beginning replacement when possible. When neurological symptoms or significant anemia are present, treatment should not necessarily be delayed while waiting for every test result. In other words, testing for B12 deficiency is not always as simple as looking at one number and asking whether it falls inside the laboratory’s reference range.
Can vitamin B12 deficiency be treated?
Fortunately, B12 deficiency is generally very treatable. The appropriate approach depends on why the deficiency occurred and how severe it is. Dietary changes may be enough when inadequate intake is the primary issue. Oral B12 supplements can also be effective, including in some people who have difficulty absorbing B12 from food because supplemental B12 is not bound to food protein. In other situations—particularly significant deficiency, certain absorption problems or pernicious anemia—B12 injections may be appropriate.
The important part is identifying the reason for the deficiency rather than simply replacing the vitamin and assuming the problem is solved.
Looking at the bigger picture
Vitamin B12 is a good example of why nutritional health isn’t always as simple as counting how much of a nutrient appears on a plate. A person can eat a balanced diet and still develop B12 deficiency because of digestive problems, age-related changes, certain medications or an autoimmune condition. Conversely, someone following a vegetarian or vegan diet may simply need a reliable source of fortified B12 or supplementation.
