Celiac Disease: More Than a Gluten-Free Diet

For someone living with celiac disease, avoiding gluten is not simply a dietary preference. It is an essential part of protecting the small intestine from an autoimmune reaction that can cause ongoing inflammation and intestinal damage.

But there is another side to celiac disease that is sometimes overlooked. Even after gluten has been removed from the diet, some people continue to experience digestive symptoms, nutrient deficiencies, fatigue, or other health concerns. Understanding what happens inside the gut — and what recovery can involve — can help make the condition feel a little less overwhelming.

What Happens When Gluten Meets Celiac Disease?

Celiac disease is an autoimmune condition in which eating gluten triggers an abnormal immune response. Gluten is a group of proteins found naturally in wheat, barley, rye, and foods made from these grains. In a person with celiac disease, the immune system responds to gluten by attacking the lining of the small intestine. Over time, this can damage the tiny, finger-like structures called villi that help the body absorb nutrients from food.

That damage helps explain why celiac disease can produce symptoms far beyond the digestive system. When the small intestine isn’t functioning normally, the effects can extend throughout the body. Celiac disease is also different from non-celiac gluten sensitivity. Someone with gluten sensitivity may experience symptoms after eating gluten, but does not have the characteristic autoimmune response and intestinal changes associated with celiac disease.

Celiac Disease Doesn’t Always Look Like a Digestive Disorder

Bloating, abdominal pain, gas, diarrhea, nausea, and unexplained weight loss are familiar symptoms of celiac disease. But they are only part of the picture. Some people experience fatigue, iron-deficiency anemia, migraines, numbness or tingling, reduced bone density, mood changes, irregular menstrual cycles, or unexplained infertility. An itchy skin condition called dermatitis herpetiformis can also be associated with celiac disease.

This wide range of symptoms is one reason celiac disease can be difficult to recognize, particularly in adults. A person may spend years trying to understand why they feel exhausted, why their iron remains low, or why their digestion never seems quite right without realizing that an autoimmune condition could be involved. For children, signs may include poor growth, delayed puberty, changes in mood, weight loss, or defects in tooth enamel.

What Causes Celiac Disease?

Genetics are an important piece of the puzzle. Most people with celiac disease carry one of two genetic variants known as HLA-DQ2 or HLA-DQ8. But having one of these variants does not mean that someone will necessarily develop celiac disease. In other words, genetics create susceptibility, but they don’t tell the whole story.

Research continues to explore how factors involving the intestinal environment and immune system may influence the development of celiac disease. The gut microbiome, previous gastrointestinal infections, antibiotic exposure, chronic stress, and other environmental influences have all been investigated as potential contributors. This is one reason a broader view of gut health can be valuable — particularly for someone who continues to struggle with symptoms after receiving a diagnosis.

How Is Celiac Disease Diagnosed?

If celiac disease is suspected, testing generally begins with blood work. Tissue transglutaminase IgA, commonly called tTG-IgA, is an important initial screening test, often accompanied by a total IgA measurement. One important point: don’t eliminate gluten before talking with your healthcare provider about testing. A gluten-free diet can affect test results and potentially make diagnosis more difficult.

Depending on the results and the individual’s circumstances, additional testing may be recommended. In adults, an upper endoscopy with biopsies of the small intestine may be used to confirm the diagnosis and identify characteristic changes in the intestinal lining. Genetic testing for HLA-DQ2 and HLA-DQ8 can also provide useful information in certain situations. However, carrying one of these genes indicates susceptibility — it does not, by itself, diagnose celiac disease.

Healing Involves More Than Taking Gluten Off the Plate

Once celiac disease has been diagnosed, a strict lifelong gluten-free diet remains the foundation of treatment. There is no supplement, digestive enzyme, or gut-health protocol that replaces the need to avoid gluten. At the same time, removing gluten is only the beginning of the conversation about overall health.

Damage to the small intestine can interfere with the absorption of important nutrients, including iron, folate, vitamin B12, calcium, magnesium, zinc, and vitamin D. Some people may therefore need nutritional assessment and targeted support while their intestinal lining recovers. It is also worth remembering that feeling better does not necessarily mean the intestine has completely healed. Symptoms can improve relatively quickly, while restoration of the intestinal lining may take considerably longer — particularly in adults.

What If You’re Still Having Symptoms?

Persistent symptoms deserve attention rather than simply being accepted as part of life with celiac disease. Accidental gluten exposure and cross-contamination are common possibilities. Gluten can also hide in sauces, seasonings, processed foods, and other products where it isn’t immediately obvious. But gluten exposure isn’t the only possibility. Other digestive conditions, food sensitivities, altered gut microbiota, or conditions such as small intestinal bacterial overgrowth (SIBO) may overlap with celiac symptoms. This is where looking at the whole person can become particularly useful.

Supporting adequate nutrition, paying attention to digestive function, managing stress, and addressing other factors that may be affecting gut health can all be part of a thoughtful, individualized approach. The goal isn’t to find a way around the gluten-free diet. The goal is to give the body the best possible conditions for recovery while continuing to manage the underlying autoimmune condition.

Living Well With Celiac Disease

A new celiac diagnosis can initially feel like an enormous lifestyle adjustment. Reading labels, preventing cross-contamination, changing cooking habits, navigating restaurants, and figuring out which foods are safe can take time. Fortunately, a gluten-free diet doesn’t have to mean a nutritionally limited diet. Fresh fruits and vegetables, beans and legumes, gluten-free whole grains such as rice, quinoa, millet, buckwheat, and amaranth, along with meat, seafood, eggs, nuts, seeds, and other naturally gluten-free foods can provide a wide range of nutrients.

Oats are a little more complicated. Although oats themselves are naturally gluten-free, cross-contamination can occur during processing. Some people with celiac disease may also react to oat proteins even when the oats are certified gluten-free. Whether oats belong in an individual’s diet is therefore something worth discussing with a knowledgeable healthcare professional. Perhaps most importantly, healing from celiac disease isn’t about finding a quick fix. It is about protecting the intestine from further injury while paying attention to the nutritional, digestive, and whole-body consequences of the disease.

Looking Beyond the Gluten-Free Label

Celiac disease is lifelong, but that doesn’t mean a person has to remain defined by their symptoms or diagnosis. With appropriate management, intestinal inflammation can resolve and the damaged intestinal lining can heal. For people who continue to struggle despite carefully following a gluten-free diet, it may be worth taking a closer look at the bigger picture.
